Job Position Details

Current Position: Family Mentor

Position Offer: Full-Time 

Job Description: Family Mentor provides mentoring and guidance assistance to improve children and family well-being, and life skills. The Family Mentor will assist with providing referrals within the community as they require resources for services such as job placement, financial, legal, housing, and medical assistance when needed. The job of the Family Mentor also requires teaching classes and one-on-one sessions to assure that clients are well guided and supported throughout every need. Roles & Responsibilities

  • Prepare and conduct intake assessments and assess their situational needs.

  • Works with families to develop a safe and healthy environment for children with the goal of keeping the family together and functioning in a positive manner.

  • Helps families that are experiencing difficulties due to job loss, life changes, mental state.

  • Empowers families to become self-sufficient and provide safe and secure environments for their children.

  • Based on assessment of client’ needs, resilience, motivation and strengths, the Mentor develops a plan identifying the action steps needed to bring about the desired changes. Develop plans to improve family and children life changes.

  • Provide a minimum of 2 classes a week. Provide parenting guidance, personal development and life skills classes, client mentoring and emotional support.

  • Participate in outreach events to recruit new client families, some of which may occasionally occur outside of normal office hours.

  • Coordinate care and services. Act as a liaison between agencies and community resources for referrals

  • Maintain pregnant/parents counseling/mentor hours as directed by the Executive Director/Program Director.

  • Support families through home visits, if needed. Deliver materials and supplies.

  • Plan regular follow-ups to assess progress and additional needs.

  • Perform all typing/computer data input.

  • Keep accurate records of client cases.

  • Provide other assistance as needed.

Education & Benefits
